Janice is a Clinical Nurse Specialist with the WRHA Palliative Care Program. In this role, she is part of the interdisciplinary consult team in palliative care, providing support to healthcare teams in acute care, long term care and the home setting. Janice has been involved in developing care plans for individuals living in the group home setting who are nearing end-of-life. She has her Master of Nursing degree from the University of Manitoba and is certified in Hospice and Palliative Care Nursing.

Session: End-of-Life and Palliative Care in a Group Living Arrangement 

Individuals living with disabilities are able to lead full and productive lives with the support of their families, direct service workers, site supervisors and clinical managers. Eventually, they will reach a point in which they are in their final months, weeks and days of life. In these situations, it is often desirable for them to spend their final days at home, surrounded by those who know them best. This presentation will discuss how we might recognize when we need to prepare for end-of-life, and what steps support the individual, families and the care team during this transition and though to death at home.
